Privacy policy

This policy covers the Skinovatio Lincoln Park website (lincolnparkskinovatio.com), our contact form, online booking, and our text messages. Skinovatio Lincoln Park, operated by 255 MEDICAL PLLC, runs this site and the clinic at 2632 N Lincoln Ave, Chicago, IL 60614 (“we,” “us”). It is written in plain English on purpose. Our Terms of service cover the text messaging program in more detail.

What we collect

• Contact form: your name, mobile phone number, email, the treatment you are curious about, the best time to reach you, anything you type in the notes, whether you checked the optional text message box, and the date and time you sent the form. The form also notes the page you came from and the ad or link that brought you to the site, so we know which pages and ads are useful.
• Online booking: booking runs through Pagoda, our scheduling and messaging system. Pagoda collects your name, mobile number, email, the service and time you choose, notes you add, your text message choices, and the card you add to hold the appointment.
• Calls and texts: the number you call or text from, the messages you send us, and any notes we take about what you asked.
• Browsing: standard analytics data such as pages viewed, device and browser type, and rough location.

We don’t ask for health details on the form or by text, and we’d rather you didn’t add them. Save those for your consult.

How we use it

To answer your questions, schedule and confirm your appointments, send reminders and updates about them, text you when you have agreed to texts, reply when you text us, understand which pages and ads people find useful, and keep the records the law requires. We don’t use your information for anything else.

Text messages and how we collect consent

SMS/Text Messaging. We collect your mobile phone number when you opt in to receive text messages from Skinovatio Lincoln Park. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties. All other categories exclude text messaging originator opt-in data and consent. Message frequency varies. Message and data rates may apply. Reply STOP to opt out or HELP for help.

We only text you if you have agreed to it. You can give that consent in two places, and each one covers different messages.

Contact form. Directly under the mobile phone field there is a checkbox that reads: “I agree to receive text messages from Skinovatio Lincoln Park at the mobile number above about my inquiry and appointments.” It also says message frequency varies, message and data rates may apply, and that you can reply STOP to opt out or HELP for help. The box is unchecked when the page loads and you can send the form without checking it. If you check it, we may text you about your inquiry and appointments. It does not sign you up for marketing texts. If you leave it blank, we will call or email you instead and won’t text you about your inquiry.

Online booking. When you book through our online booking portal, run by Pagoda for us, the booking step asks you to agree to appointment texts: confirmations, reminders, changes or cancellations, and requests for information we need before your visit. Promotional texts about offers are a separate, optional choice in the same step, and they are never a condition of booking. If you would rather not receive appointment texts, call us at (773) 799-8278 and we’ll book you by phone.

What we store about your consent. Your mobile number, which box you checked, the wording you agreed to, where you agreed (contact form or booking), the date and time, the texts we exchange, and whether you have opted out. This is kept in Pagoda and in the website’s form records. We use it only to send the messages you agreed to, to stop texting you when you opt out, and to show that you gave consent if we are ever asked.

Message frequency varies. Message and data rates may apply. Reply STOP to any text to opt out, or HELP for help. Consent to texts is not a condition of any purchase. Please don’t send medical details by text.

Who sees it

• The clinic team who books and treats you.
• Pagoda, which runs our booking, card hold, and text messages for us.
• Affirm, Afterpay, or Klarna, only if you apply for financing, and only what you give them directly.
• Our website hosting, email, and analytics providers, which help us run the site. Analytics tools see site usage, not your form answers.
• Authorities, only if the law requires it.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties. These exclusions apply to every group listed above.

What we don’t do

We don’t sell your information, including your phone number and your text message consent. We don’t hand your name, phone, or email to advertisers. We don’t require a marketing opt-in to send the form or to book. We don’t share your details with the other Skinovatio offices unless you ask us to.

Health information

What you tell a provider in a treatment room, your treatment record, and your before and after photos are kept in the clinic’s records system, separately from the website, under clinical privacy rules. Photos appear on this site only with your written permission, and you can withdraw it.

Cookies and analytics

The site uses cookies for the things a site needs to work and for analytics. You can block cookies in your browser and the site will still load.

How long we keep it

Form messages are kept as long as we need them to answer you and follow up. Consent records are kept while you are on our text list and for a period afterward, so we can show when and how you agreed. Booking, payment, and treatment records are kept as long as the law requires.

Your choices

Reply STOP to any text to end texts. Ask for a copy of what we hold on you, ask us to correct it, or ask us to delete it, and we will, unless a booking or legal record requires us to keep part of it. Children

This site and our services are for adults. We don’t knowingly collect information from anyone under 18, and a parent or guardian can ask us to remove it.

Changes

If this policy changes, the effective date at the top changes with it, and anything that affects how we use what you’ve already given us will be explained on this page before it applies.
