How long do PDO threads last, and what happens when they dissolve

For many people, the lift from PDO threads holds for several months to about a year, although manufacturers market 12 to 18 months. The thread itself dissolves over months, and the result you see is the collagen your skin builds along it. That’s why the peak comes at month three, not day one.

So there are two clocks at work, and confusing them is a common source of disappointment with threads.

Two clocks: the thread and the collagen

A PDO thread lift places absorbable polydioxanone sutures under the skin through a cannula. Barbed threads grip the tissue and hold the cheek or jowl slightly higher from the day they go in. Smooth and twist threads are laid in a mesh to build collagen without lifting.

The first clock is the thread. PDO is a surgical suture material, and your body breaks it down over months. As it goes, the tissue around each thread forms a fibrous, collagen-rich track, per a 2025 systematic review of 12 studies and 818 patients.

The second clock is that collagen. It builds over 2 to 3 months, which is why your face looks better at month three than at week one. Then it ages like any collagen, so the lift softens over the following months. The collagen is the result you’re paying for, and the thread is what starts it.

What the evidence says, in numbers

Real-world results vary more than brochures suggest. Patients on RealSelf report that the initial lifting effect can fade in six months or less, and 6 to 12 months of lift is the common report. Manufacturers market 12 to 18 months.

The published data sits in between, though it doesn’t follow people for very long. The 2025 review found scores improved in every study immediately after the procedure, complications were mild, the longest follow-up was 2 years, and “long-term efficacy data are lacking.” A 2021 meta-analysis of 26 studies saw satisfaction fall from 98 percent immediately to 88 percent later and advised telling patients the effect may not hold long term.

One small combination study of 11 patients who had barbed threads plus hyaluronic acid filler measured contour changes still present at 24 months. Eleven people is a small group, but it’s the longest follow-up available. Individual results vary.

What “dissolve” means, and what stays

Nothing needs to be removed. The PDO is absorbed and the collagen track stays. So PDO threads do leave something behind: the collagen that holds the result.

Some side effects can also persist. In the 26-study pooled data, skin dimpling ran 10 percent, an odd or numb sensation 6 percent, a thread you could see or feel 4 percent, and extrusion 2 percent, with absorbable threads scoring better than non-absorbable on the last two. If you notice puckering along a barbed thread on day one, that’s expected and should settle within a week. Feeling a thread over the cheekbone in week two is common too, and it usually fades as the material softens.

Older tissue behaves differently. Dimpling ran 16 percent in older patients versus 5.6 percent in younger ones in that meta-analysis. That’s one reason the candidate range we look for is 35 to 55, with adequate skin thickness.

Making it last longer: maintenance and Morpheus8

Three things can extend the useful life of a thread lift.

The first is protecting it in week one. Sleep on your back with your head raised, and avoid wide yawns, chewing gum, and tough foods like steak. It’s best to skip facials or massage for 2 to 4 weeks, and hard exercise for a week. A barbed thread that’s tugged before the tissue has settled around it loses its grip early.

The second is skin quality. Threads reposition tissue but don’t change the skin over it. Morpheus8 contracts and remodels the skin with radiofrequency heat over three sessions, and the two are often paired: threads for position, Morpheus8 for firmness. Threads peak at about three months, and a Morpheus8 series reaches its full result about three months after the last session. Whether Morpheus8 makes the threads themselves last longer hasn’t been measured, but it keeps the skin over them firmer for longer.

The third is a repeat treatment. Placement can be repeated at about 12 months or whenever the lift has faded, and many people find the second round holds better because there’s already a collagen track to build on. That’s a clinical impression, not a trial result.

Timing a thread lift before an event

If you have an event coming up, plan the treatment six to eight weeks ahead. Swelling resolves in about a week and tenderness in two or three days, but the collagen needs 2 to 3 months to make the lift look natural rather than pulled. Booking eight weeks ahead gives the result time to settle before the date.

Do PDO threads leave anything behind?

Yes, and it’s collagen. The polydioxanone suture is absorbed over months, and the tissue around it forms a fibrous, collagen-rich track that holds a modest lift and firmness after the thread is gone. Nothing is removed and nothing foreign stays. Occasionally a small dimple or a thread you can feel persists longer than expected, and prior threads can make a later facelift more complex, so it’s important to let a surgeon know about them.


Can I repeat a thread lift after a year?

Yes. Repeat placement at about 12 months, or when the lift has visibly faded, is the usual rhythm. The second round goes into tissue that already has a collagen track from the first, and many people find it holds well. Smooth-thread mesh treatments for texture are sometimes done as a set of 2 to 3 sessions rather than one. Does Morpheus8 make threads last longer?

That hasn’t been measured directly. What Morpheus8 does is tighten and remodel the skin over the repositioned tissue with fractional radiofrequency across three sessions, so the overall result looks firmer for longer. Threads take care of position, and Morpheus8 takes care of skin quality. Threads peak at about three months and Morpheus8 about three months after the final session, so they’re often planned as a pair.


Why does a thread lift look better at month three than at week one?

Because week one is swelling over a mechanical reposition, and month three is collagen. The barbs move the tissue on the day, then swelling and sometimes puckering hide it for about a week. Over the next 2 to 3 months the body builds collagen along each thread, which softens the pull and adds firmness. Judge your result at month three, not in the first week.
